Two strange glitches with GW
I want to mention two odd things that have happened to me in the last week.
1) While doing the very end of the Hell's Precipice mission, where you have to kill the Lich on the Bloodstone, I had the strange experience of being Teleported into the lava!
I wasn't moving and did not have any teleport spells on me at the time. At first, I though it was a lag issue. So I made my way out of the lava and resume attacking the Lich. A few seconds later, POW, I was in the lava again, but on the other side.
I know the Lich, himself, normally teleports into the lava. But he never teleported during this mission. So I am guessing that it was He who was transporting me out there, somehow instead of himself.
Anyone else seen this? Is it a bug?
2) While running a 55 Necro in UW, I got Rended and died. But when he monk Rezed me, I immediately died. Over and Over.
I thought they fixed this, so you rezed wih 1hp minimum?
In fact, I died several times earlier during the trip and DID rez with 1hp. After the earlier deaths, I cured my DP with a Candy Cane.
But when I died and couldn't be Rezed, I could NOT change Cestas NOR eat a CC. Not sure what was happening. Thoughts?

1. It is a Lich skill. You switch places.
2. DP. With DP penalty you dont have enough to be alive.

For #1, when I did the last mission, our Warriors did get teleported very often.
As for #2, on my 55 Monk, I res with 1HP no matter how much DP I have. You must be bugged.

1 is correct, but Carinae is right on 2 - you should always be rez'd with 1hp regardless of DP.

55 build will not res, is normal unless its a 100% health res. usually res sig or rebirth. I have always experienced this instant death. Why i guess the dp.
At res shrines ya get 1hp always if you experience problem then i think you got a glitch happening to you somewhere.
however I Beg you please no arguing the issue of 55 build. It just seem the more ya mention one of the solo builds and what problems they have the more they do what they can to make the 55 style build harder to use.
as to the lich i have never experienced it no idea on how to answer the lich problem.

Saider is correct. The res must give you 100% health or DP penalty will kick in and kill you at the spot.

Ahhh. Interesting. The monk (Savio) was using Rebirth, which resurrects at 25% health.
But I died several times earlier (I think) and was resurected successfully. Savio, did you use a different resurrect on me then?

It all depends on the rez skill used. if you use rez sig or veangence/unyeilding, it will rez you no matter your hp. However if you try skills that dont give you back all of your hps, rebirth/restore/resurrect etc.,then you just die over and over.
Easy way to overcome this is for someone to bring vengeance and rebirth. When the 55 dies cast vengeance to give the person a chance to switch focus/armor then rez him properly when veng runs out.

FYI: It was the strangest 55 build evar!
6 casters, no tank. Several people, including me, wanted spiders. Only I had the stuff to run as a 55, but I had just changed to /R (for the spider), so I couldn't protect/mend myself.
I was BiPing the Monk, from inside a pile of Ataxes(!!!) so he could spam PS/HB on me, since he couldn't see when PS was ending!
The other 4 people, were responsible for actually killing the bad guys.
Necro draws and maintains aggro.
Monk protect/heals necro.
Necro BiP's Monk while under bombardment.
Everyone else kills.

The lich thing, well I first learned about that from the strategy guide. It's a skill/spell/ability that he uses when people attack him (can't remember the full details). It recommends that you be prepared for it with some kind of healing as you have to wade through lava to get back to him. He'll also teleport himself if the going gets tough. You also usually have to kill the lich multiple (3 I think) times to defeat him. Although, when I did it the first time (with only henchies), I could've swore the strat guide was wrong and I only killed him once. The second time (with a PUG) we had to kill him three times, but it may have something to do with where he dies (someone said bloodstone). Anyhow, the strat guide is dated and wrong in many places. As far as the rez thing. That's quite common. If it's a 100% rez (including shrine) then you'll come back at 1hp (regardless of how much DP). If it's less, you really are not rezzed (looks like dying again but the dp doesn't change and you can't change any items). This causes most pairs to bring along vengeance on the second character (the spitey necro or what not) so that he can rez you at 100% just long enough to switch out armour/offhand/etc and get back to ~55 before you die again (30 seconds with vengeance). THEN, he rezzes you with whatever normal (hopefully rechargable) rez that he has. I hope that helps.
EDIT: lol, during the time I was writing this reply I guess I was beat to the punch by like 3 other messages (at least on part of it)..

Undead Lich uses a Monster only skill called "Soul Vortex" and if you watch closely, you can see the vortex coming towards you if you are on the stairs, right in front of the bloodstone. It falls in the same category as skill like "Claim resource", "Giant Stomp", "Nibble", "Spectral Agony" etc....

Well, actually, it's something like this:
You died enough to have a 60% Death Penalty. Please bear with me and my calculations and my abstract theoreticism.
.6(480hp) - 5 Superior Runes(-75) + -50hp offhand = -87hp
At that point you're color changed from blue to red, essentailly a negative of yourself. With Savio's Divine Favor attribute at 12 you would pass from the negative you into the positive you in 3 casts. He would cast his ecnchantment and bond, then one more enchanment and, "Poof!". The GW servers couldn't handle the complex spiritual issue....so GW just killed you.

1. Posters are correct, lich teleports people at random into the lava (this is why I use absolutely NO hench on the last mission, they just stand around in the lava and look confused ... well, maybe it's more than "looking" ...)
2. It has to do with your DP, other posters are correct. Since you can't switch offhands or armor (or for that matter, do anything ... you're DEAD! ) a full 100% life res will bring you to 1HP, and anything that restores you to "25% life" or "62% life" will not work, as the game rounds fractions down, and thus you are at 0 life (25% of 1 is .25, rounded down is zero). Another poster mentioned bringing vengeance, which is a superb idea, as it will give the 55 monk a chance to put on normal armor (or just take the superior armor off) so they can be res'ed normally after vengeance wears off. Unfortunately for 2 man UW teams, Vengeance is a monk skill.

Actually a 16 healing prayers monk with restore life (resses with 52% hp), does in fact res even if you have 1 hp. Restore life doig 50% or lower will not res you.
The game will round up if the % is above 50%, the game will round down if it is 50% or lower. ie: normal ressurect spells or 15 healing prayers restore life.

On a side-note you can teleport the Lich into the lava, by running towards/past him when one of the teleporting-blinky-thinga-mabobs is following you.
